About this paper

This is the fourth and final Canadian market I have scored across banking, payments, and now health — and Canada has been the same shape every time: small, concentrated, province-fragmented, coordinated federally only at the standards level, and thin where a developer or an agent actually needs a contract. This report scores ten Canadian healthcare organizations the way an agent would read them, and it is the lowest-scoring market in the entire healthcare series.

The finding is a triple inversion: Canada has no FHIR-native challenger class at all, so the most API-native player is Jane — a niche practice-management SaaS, not an incumbent and not a FHIR startup. The full report maps a market with zero live clinical FHIR endpoints, gated incumbents, a terminology-only federal steward, and one vendor proving that agent-aware API design wins even from an unexpected corner — a whitespace the size of a country, with the seat for a real clinical-FHIR platform sitting empty.

What's inside

  1. Executive summary — fragmented, gated, and missing a challenger class
  2. The state of the sector — fragmented, gated, and missing a challenger class
  3. The resource taxonomy — one rich proprietary API, and a lot of nothing
  4. The composite Kin Score — and why it's the foundation
  5. Agent-readiness — one legible surface, no native one
  6. The blueprint — what the leader satisfied, and what nobody has
  7. Standards and artifacts — federation without a front door
  8. Security, auth, scopes, and consent — heterogeneous, and never SMART
  9. Provider-by-provider intelligence — the outsider, the gated giants, and the steward
  10. The investable thesis — a whitespace the size of a country
